Abstract

The six commentaries provide valuable insights from a range of perspectives. Some explore a particular aspect of my paper in greater detail. Others take a wider view, exploring the health system as well as historical, political and international contexts. In this response, I note salient points and common themes and offer my views on new issues raised.
